Position Responsibilities

  • Managing Treatment Plans: As a Nurse Practitioner, you will be responsible for developing and adjusting treatment plans and medication for patients, particularly those with chronic conditions such as Heart Failure (HF), Type 2 Diabetes (T2D), and Hypertension.

  • Conducting Virtual Check-Ins: Regular virtual check-ins with patients are part of the role, where you will help optimize patient adherence to guideline-directed medical therapy (GDMT), review lab results, and adjust treatment regimens accordingly.

  • Contributing to Care Quality: Every healthcare interaction is considered an opportunity to shape the patient’s experience with Cadence’s services. You’ll be instrumental in implementing best practices that prioritize patient welfare aligned with the health system's goals.

Required Skills and Qualifications

To qualify for the Nurse Practitioner position, you must possess the following qualifications:

  • Experience: A minimum of 5 years of experience treating patients with chronic diseases, either in outpatient or inpatient settings, preferably at high-performing medical centers.

  • Licensure: Candidates must have an active Compact Multi-State RN license, along with an Active Nurse Practitioner (NP) license in at least one of the following states: Texas, California, New York, New Jersey, or Hawaii. Preference will be given to candidates with licenses in multiple states.

  • Education: A Master's Degree as a Nurse Practitioner is required, and ANCC or AANP certification must be obtained.

  • Technical Skills: Familiarity with remote patient monitoring technology is preferred, along with the capability to thrive in a remote work environment, which requires a consistent and strong Wi-Fi connection.

  • Patient-Centric Attitude: Demonstrated passion for patient care, independent problem solving, and innovation in healthcare delivery are essential traits.

  • Preferred Experience: Experience working in a CHF bridge clinic or T2D clinic is desirable, as well as working in a startup environment.
